Much of the information on the form is required and must be entered before you can submit the form. Please make sure to include any required information before clicking the submit button.

You will receive an email confirming that your request has been submitted. You will also receive an email when the event has been approved by a scheduler, or if the event is not approved. YOU MAY NOT ADVERTISE OR PLAN ON USING FACILITIES UNTIL YOU HAVE RECEIVED APPROVAL.

SERVICES REQUESTED

INDEMNIFICATION: Tenant agrees to wholly indemnify, reimburse and hold Landlord harmless from any and all liability and damages which may be incurred on the Property and/or Premises during Tenant's period of possession arising out of Tenant's use and occupancy, as may be caused or arise from the acts, or failure(s) to act, of Tenant, its clients, guests, licensees, agents and invitees.

INSURANCE INFORMATION

All non-profit and for-profit users of school facilities are required to file a certificate of insurance, along with their facilities usage form, with the District Clerk, Laurie Cooper (lcooper@svecsd.org). Insurance documentation is to be filed prior to the use of the facilities and shall contain the following dollar amounts:

Bodily Injury and Property Damage $500,000 Each Occurrence
$1,000,000 Aggregate
Premises Medical Expenses $1,000 Each Person
$25,000 Each Accident
Products/Completed Operations $500,000 Each Occurrence
$1,000,000 Aggregate
Fire Legal Liability $50,000 Each Occurrence
Personal Injury and Advertising Liability $500,000 Each Occurrence
$1,000,000 Aggregate

You are responsible to provide Proof of Liability Insurance to the Secretary to the Superintendent at lcooper@svecsd.org within three (3) business days of submitting this request.

 

You may also be responsible for support staff expenses as a result of facility use.

All persons, organizations, or other entities that request the use of district facilities must be in compliance with Title IX of the Education Amendments of 1972.

  I, as the agent of the organization making this request, agree to comply to the extent applicable with Title IX of the Education Amendments of 1972 and applicable requirements imposed, to the end that no person shall be excluded from participation in, denied the benefit of, or be otherwise subjected to discimination under any activity conducted by the group/organization while using school facilities.
